We have implemented an online system for all requests for health records. This system will improve our ability to fulfill requests in a timely manner and ensures you receive information in an electronic format.

For Third Party Requests for Records: There are two ways to request records using the electronic system:

For Patient Requests for Records:

Just follow this link: Patient Request to submit your request for health records and everything happens through a secure online portal. Once you’ve made your first request, you’ll have Medchart account created to track and receive your records. Any additional requests can be made right from your personal account.

Your privacy and security are our number one concern. That is why we have implemented this new online system that is more secure than fax machines and physically mailing records, while also giving you easier and faster access to your own health information.

There is no cost to setting up a Medchart account and it provides many benefits for businesses including a centralized portal for all your medical record requests, optical character recognition on released records, enhanced invoices, trusted authorizations, etc.

We strongly recommend using our enhanced system for requests. If you choose to fax or mail a request, we will ensure this is your intention. Manual requests may incur longer than expected delays and a higher rate of rejection.
